NHS ordered to reveal data of avoidable deaths

The Department for Health has requested that the NHS reveal data on figures where death could have been avoided / prevented. It has been estimated that there are up to 9,000 avoidable deaths caused by NHS failings. At the current time there is no standardised definition of “avoidable death”. The Department of Health want the NHS to learn lessons from each case. The data will include details on investigations and reviews into deaths whilst under NHS care.
